By checking this box, I confirm that the BridgeUSA National Team is NOT responsible for booking EITHER my travel or my departure to the BridgeUSA National Summit in Philadelphia. I am submitting this form as an exemption to the provided Air Travel either to the event. 

By checking this box, I hereby acknowledge that it is my responsibility to book my own travel as I've specified in previous pages of this form. 

By checking this box, I hereby acknowledge that it is my responsibility to submit my travel expense receipts for reimbursement once the proper channels to do so are provided by the BridgeUSA National team. 

By checking this box, I herby acknowledge that BridgeUSA will reimburse up to $400 in travel expenses with receipts provided for my purchases. I am prepared to be responsible for all additional costs.
